Why Immunotherapy Is Cheaper in India — and What That Does and Does Not Mean

Immunotherapy drugs are patented biologics manufactured by pharmaceutical companies in the US, Europe, and increasingly Asia. In Western markets, pricing reflects research and development costs, regulatory approval costs, and market pricing power. In India, several factors reduce the cost significantly:

  • Hospital procurement pricing: Indian hospitals negotiate lower drug procurement prices than individual patients buying retail. The per-cycle costs in the table above reflect hospital procurement rates, not retail pharmacy prices.
  • Biosimilar availability: For monoclonal antibodies like trastuzumab and bevacizumab, Indian manufacturers have produced biosimilar versions — drugs that are clinically equivalent to the originator but manufactured off-patent. These cost 70–90% less than the originator brand. Checkpoint inhibitors (pembrolizumab, nivolumab) do not yet have approved Indian biosimilars; these are still originator products, but procured at lower Indian market rates.
  • Lower hospital operating costs: Administration fees, nursing costs, and day-care facility costs for chemotherapy and immunotherapy sessions are lower in India than in Western hospitals, reducing the total per-cycle cost beyond just the drug price.

What this does not mean: the drug itself is not different. Pembrolizumab administered in Mumbai is the same molecule as pembrolizumab administered in London.
